As you may be aware, Cypriot Government issued Circular No. 54/2004, attached for your quick reference, on 16 December 2004 in order to totally revise the Forms for "Declaration of Particulars for the ISM Code".

The Forms referred to in the Circular No. 13/2002 (dated 29 May 2002), I.e. Form ISM. 01/Revision 01, Form ISM. 02/Revision 01 and Form ISM. 03/Revision 03, were cancelled and replaced by the revised form, I.e. Form ISM. 01/Revision 02.

The Circular No. 54/2004 is available from the Web Site (http://www.shipping.gov.cy/) of the Cypriot Government.
The revised ISM Declaration Form, I.e. Form ISM. 01/Revision 02, is also available from the ClassNK's Web Site (http://www.classnk.or.jp/hp/topj.asp).
