In order to simplify the formalities to request class surveys of new-building ships, an application form 1A for CLASSFICATION AND STATUTORY SERVICE DURING CONSTRUCTION has been amended as attached hereto.

ClassNK shall be very grateful if you will use this new form in future, which may be down-loaded from the following URL.

At the occasion to make an application, it is enough to send us Form 1A, followed by the documents as Supplements to Form 1A, which may be submitted later but prior to relevant plan examinations or surveys.

Supplements:
1. Form 1A-1: Certificates and main particulars
2. Form 1A DG-BC : Application for DG certificate and BC code fitness certificate
3. Form 1A CG : Particulars of Cargo Gear Derrick / Crane
4. Form 1A C : Application of various attestation

It is also accepted to send us Form 1A and its Supplements concurrently.

It is possible to omit said Supplements in case where any sister ship(s) with same specific design classed by ClassNK is available.
